Personal Statement

I understand that STAR DANCE ACADEMY, its management, staff, instructors and voluntary support parents will exercise all due care, but will not be liable for any injury or damage which may sustain to person or property and in case sickness or accident.

If considered advisable I request MEDICAL attention be secured at my expense and notification sent to me.

It is recognized that there may be some hazards in both indoor & outdoor activities and it is the responsibility of both the instructor to eliminate or minimize these hazards.

Photo Release & Video Footage Release
In agreeing to these terms and conditions I agree and understand that any photos and video footage taken of my child/ myself is the copyright property of Star Dance Academy.

I agree that any photos or video footage of my child/ myself can be used for promotional purposes. I waiver the right for any financial returns for the usage of any photos of my child/myself. I have the right to ask for the removal of a photo or video if I deem my child/ myself is not promoted in a positive or manor, or I feel un-comfortable or embarrassed by a certain image be that photo or video footage.
